The following immunizations are required before your child begins Kindergarten:

• TDaP/DTP/DT/TD: 4 Doses (4th dose on or after 4th birthday)

• Polio: 4 Doses (4th dose on or after 4th birthday)

• Hepatitis B: 3 Doses (Last dose on or after 6th months of age)

• MMR: 2 Doses

• Varicella: 2 Doses (or documented case of chicken pox disease)

The following immunizations are required before your child begins seventh grade:

• TDaP: 1 Dose of Tetanus, Diphtheria, acellular Pertussis

• MCV: 1st of 2 Doses of Meningococcal Conjugate Vaccine

The following immunizations are required before your child begins twelfth grade:

• MCV: 2nd of 2 Doses of Meningococcal Conjugate Vaccine
