  • Family Dentist Report

    The Pennsylvania School Health Law requires children to have a dental exam upon original entry to school, in the third and seventh grades, and when a student is transferring to Manheim Central.  Attached is a Family Dentist Report Form to be filled out when your child receives his/her dental exam.*

    *The district will accept medical/dental reports completed within one year prior to a student's entry into the grade where an exam is required.

Medical Exam

  • Physical Examination Form

    The Pennsylvania School Health Law requires children to have a complete physical exam upon original entry to school, in the sixth and eleventh grades, and when a student is transferring to Manheim Central. Attached is the Private Physicians Report Form to be filled out when your child receives his/her medical exam.*

    *The district will accept medical/dental reports completed within one year prior to a student's entry into the grade where an exam is required.

Athletic Participation

  • A physical exam is required before a student participates in district athletics. The exam can either be with your family physician or with the school doctors during the district’s scheduled physicals.
