Parent Resources

  • For those that have submitted exemption requests, we have vetted all  requests through our solicitor and are responding as to the status of your request.  As we have previously indicated, documentation of a medical or mental health condition or disability provided in support of mask exemption must be in accordance with Section 504 of the Rehabilitation Act or IDEA. The “Heritage” form or parent exemption note does not meet this requirement. The form below is an example of the information we need to process a medical exemption.  This information is required before September 20, 2021.  If you require a temporary exemption pending your doctor’s appointment, please complete this form
